Your success will come from knowing what employees value in regards to healthy living and their outside-of-work activities & interests.  With this information you can deliver wellness activities designed to increase participation and be programs your employees rave about! 

By using a third party vendor – like us – to collect the information, employees feel their responses are confidential and are more likely to give accurate and honest information.

At You Define Wellness, we offer such a service.  We will…

  • design a custom survey with questions you can suggest;

  • distribute the survey;

  • tabulate the results;

  • analyze the data; and,

  • provide you with a custom report and recommendations. 

This includes a one-hour consultation to review the recommendations AND 50 Healthy Living Savings Cards you can use as incentives for your staff.  

This is a $7,450 value; it’s available to you for the rock bottom price of $2,997.

Refund Policy

All sales are final with the exception below.

For the Wellness-in-a-Box programs, we will refund the unused Wellness Talks portion of the plan less administrative fees within 30 days of your written notification of cancellation.

Cancellation / Return / Exchange Policy

To cancel any recurring program such as the Employee Wellness Benefit Plan, simply notify us of your intent to cancel or don't pay the next invoice.  An unpaid invoice as of its due date will automatically terminate benefits for your employees and it is your responsibility to notify employees as such.

To cancel remaining Wellness-in-a-Box benefits at any time during its 12-month period, simply notify us in writing of your intent to cancel.
