A successful workplace wellness program comes from knowing what employees value in regards to their healthy living… and an analysis of their outside-of-work activities & interests is a good place to start! With this information, you can deliver activities designed to increase participation and be THE workplace wellness program employees rave about!

When you select You Define Wellness to conduct the employee survey, employees feel their responses are confidential leading to more accurate and honest information.

We will…

  • Design a custom survey with your input;
  • Distribute the survey;
  • Tabulate the results and analyze the data; and,
  • Provide you with a custom report and recommendations!

This price includes as many consultations as necessary to develop the survey, a one-hour consultation to review the recommendations AND a Healthy Living Savings Card* for each participant ($99 value).

Refund Policy

All sales are final with the exception below.

For the Wellness-in-a-Box programs, we will refund the unused Wellness Talks portion of the plan less administrative fees within 30 days of your written notification of cancellation.

Cancellation / Return / Exchange Policy

To cancel any recurring program such as the Pathways: Wellness@Work Option, simply notify us of your intent to cancel or don't pay the next invoice.  An unpaid invoice as of its due date will automatically terminate benefits for your employees and it is your responsibility to notify employees as such.

To cancel remaining Wellness-in-a-Box benefits at any time during its 12-month period, simply notify us in writing of your intent to cancel.
