  • Your benefits program will launch with a complimentary Employee Needs Analysis (up to $550 value);
  • Monthly group wellness education for all employees from our Wellness Talks catalog (up to $250 value) plus Education Coordination Services from You Define Wellness are included;
  • Each enrolled employee will receive monthly a credit up to $60 to be applied to any service offered by You Define Wellness network providers who accept the Employee Wellness Benefit Plan;
  • Each enrolled employee will also receive a Healthy Living Savings Card (a $99 value)This card never expires, can be shared with family members, and used multiple times in a month for additional savings on out-of-pocket healthy living products and services; plus,
  • We will organize annually an exclusive virtual or live Healthy Living Fair with many of our network providers.  Enrolled employees may attend for free; not-enrolled employees will incur a fee of $20 per person.

Price indicated is per enrolled employee, per month.  When signing up online, select the number of employees to participate and complete the payment information.  Upon receipt, a representative of You Define Wellness will contact you to coordinate details.  Although we ask for a 1-year commitment, this is a month-to-month plan and you will be invoiced monthly; cancel at any time.

This employee benefit program is best suited for groups of 10+ employees.

Employer Agreement

We hereby agree to offer the You Define Wellness (YDW) EMPLOYEE WELLNESS BENEFIT PLAN for the benefit of our employees.  The YDW plan consists of one wellness session per enrolled employees and one wellness education class for all employees monthly unless customization has been requested for as long as our account is in good standing.

We agree to remit to YDW the wellness program fee for all enrolled employees prior to the 1st of each month that activates new sessions; all paid benefits are available to employees as of the 1st of each month as long as payment has been received by YDW.  We understand that failure to remit payment in a timely manner may suspend or terminate services for employees. We understand provider payment for services received by enrolled employees will not be advanced by YDW awaiting payment from us.

We agree that this program will remain in effect for one year from the program start date and will automatically continue as of the renewal date indicated above and all subsequent years until terminated in writing by us.  We understand that the rates stated in this Agreement are valid for the initial plan year only; the rate may be adjusted by YDW prior to subsequent plan years.

We understand this program is not subject to COBRA or State Continuation but have the right to offer it as a benefit to former employees in accordance to our internal applicable procedures.  We understand it is our responsibility to notify YDW of terminated employees.

We acknowledge that our account will be credited with one-half of unused and expired session values within 30 days of said session expiration.  Session value is computed after deduction for administrative and sales expense.  The remaining one-half of unused and expired session values will help support non-profit healing partners in the community.  We agree YDW will designate the recipient organization although we reserve the right to select a non-profit of our choosing.

We acknowledge that the YDW program has been explained in full, that YDW is not an insurance program nor is it a replacement for traditional medical insurance, and we fully accept and subscribe to all the terms and conditions contained in this Agreement.

Refund Policy

All sales are final with the exception below.

For the Wellness-in-a-Box programs, we will refund the unused Wellness Talks portion of the plan less administrative fees within 30 days of your written notification of cancellation.

Cancellation / Return / Exchange Policy

To cancel any recurring program such as the Employee Wellness Benefit Plan, simply notify us of your intent to cancel or don't pay the next invoice.  An unpaid invoice as of its due date will automatically terminate benefits for your employees and it is your responsibility to notify employees as such.

To cancel remaining Wellness-in-a-Box benefits at any time during its 12-month period, simply notify us in writing of your intent to cancel.
