Description

WHAT’S INCLUDED:

Healthy Living Sessions:

  • One session per frequency selected from services listed by our provider network.  Some services will require an additional fee payable to the provider.
  • Healthy Living Savings Card for discounts on other products and services.
  • Employer report of utilization based on frequency selected; reports ensure confidentiality of employee with all potential identifying information omitted.

Individual Well-Being Assessment + One-on-one Coaching:

  • Each enrolled employee will take an online well-being assessment followed by a 90-minute one-on-one coaching call with a Wellness PathFinder from our network.
  • Quarterly sessions:
    • Enrolled employees will receive 3 additional quarterly 60-minute one-on-one coaching calls with a Wellness PathFinder.
    • Enrolled employees will be asked to complete another assessment at the 6-month mark.
  • Semi-Annual sessions:
    • At the six-month mark, enrolled employees will be asked to complete another assessment and will receive 1 additional 60-minute one-on-one coaching session with a Wellness PathFinder.
  • Employer report of well-being assessment; reports ensure confidentiality of employee with all potential identifying information omitted.

Group Wellness Education:

  • A group wellness education class of employer’s choosing will be available with the frequency selected.
  • Concierge coordination services provided by You Define Wellness.
  • Participant evaluation and survey including post-event report to employer.
  • The total number of enrolled employees will be used as a base number.  Any additional participants above this number will result in a fee of $15 per additional person payable to the provider.

Note: certain benefits are only available through our robust employee benefit plan for healthy living, Pathways: Wellness@Work; Pathways is available for purchase for $146 per enrolled employee, per month.  These include an annual Wellness Fair, the employee survey, and a review of wellness options in other benefit programs you may own.

Guidelines

INITIAL SET UP

  • Follow the link to complete the Employer Agreement.
  • You will be invoiced upon acceptance; payment is due prior to the initial effective date.

BILLING

  • We bill in advance.  You Define Wellness will invoice by the 10th of each month for the subsequent month; payment is due by the 25th.  Failure to remit by the 25th may delay services for your employees.
    • Selecting the quarterly frequency for healthy living services: we require 3-months paid prior to effective date.

PARTICIPATION & CONTRIBUTIONS

  • We require at least 5 employees enroll on the initial sign-up.
  • We suggest employers contribute at least 50% of the cost of the program.
    • If cost-sharing with employees, we do NOT recommend pre-tax employee contributions.

OBTAINING A QUOTE

  • We do not price our program based on enrolled employees as our utilization is factored on the back end. As this is unusual, let us explain:
    • As employees earn sessions with our providers based on the frequency selected, each session code is valid for 90 days.
    • If an earned session expires in 90-days without being used, 1/2 of the value of that session will be returned to the employer.
    • The other 1/2 value is paid to a non-profit healing partner in the community; this can be a non-profit organization the employer support.

Refund Policy

All sales are final with the exception below.

For the Wellness-in-a-Box programs, we will refund the unused Wellness Talks portion of the plan less administrative fees within 30 days of your written notification of cancellation.

Cancellation / Return / Exchange Policy

To cancel any recurring program such as the Pathways: Wellness@Work Option, simply notify us of your intent to cancel or don't pay the next invoice.  An unpaid invoice as of its due date will automatically terminate benefits for your employees and it is your responsibility to notify employees as such.

To cancel remaining Wellness-in-a-Box benefits at any time during its 12-month period, simply notify us in writing of your intent to cancel.
